Preparation for Adulthood – Good Health
Young people are supported to manage their own health as they move into adulthood.
We have heard from families and young people that moving between children and adult health services can be challenging.
For children and young people with SEND in Southwest London we are working to improve our processes.
This includes improving information that is available for young people and their families, supporting the uptake of annual health checks for those with learning disabilities and clarifying what is available from different services after 18 years.
Health services work in partnership with families, education and social care to consider the needs of young people as they move towards adulthood. This will include working together to consider the outcomes on young people’s EHCPs to reflect Preparation for Adulthood in terms of employment, independent living, community participation and good health.
Annual Health Checks
All children and young people should be registered with their local GP. Young people with learning disabilities who are over 14 years old should be registered with GPs and placed on the GP learning disability register. This will mean they are then offered an annual health check. This takes place once a year, every year after young people turn 14, to ensure people stay healthy. The GP or Practice Nurse will make contact to arrange this.
