Community Learning Disability Health Team (CLDHT)

If you have a learning disability or know someone that does, and they have difficulties accessing mainstream health services, Wandsworth's Community Learning Disability Health Team (CLDHT) may be able to help you. 

They are a specialist team of:

  • Community Nurses
  • Dietitians
  • Dysphagia Practitioners
  • Exercise Practitioners
  • Occupational Therapists
  • Physiotherapists
  • Psychiatrists
  • Psychologists and and Challenging Needs Practitioners
  • Recovery Support workers and
  • Speech and Language Therapists.

In general, people with learning disabilities should be accessing mainstream services.  However, there will be times when this is not possible or appropriate and specialist services need to be involved.

Anyone can refer to the CLDHT.  Individuals may self refer, and, if you are making a referral for someone else, wherever possible, please seek their permission.  You can phone between 9:30am and 4:30pm, Monday to Friday.  You can ask for a referral form to be sent to you, or download and print out the one attached.  You can fax or post the completed referral form to the fax numbers or address below.

The CLDHT does not provide an emergency response services.  Where emergency health care is required, contact your GP or call 999.
