Young people have told us that the correct level of 'joined up' health support is essential to ensuring that they can reach their goals of getting a job, accessing the community and living independently.

So it is essential that health professionals are involved in all discussions regarding plans for adulthood.

Your GP If you are a young person with physical or learning disabilities it is important to think in advance of turning 18,  about how you will need your GP to support you to achieve your life goals.

Other Health Services Some health services you receive as a young person may no longer be available to you once you are over 18. It is important that professionals involved in your care before you are 18, pass on information to your GP and any other health services you may need after you are 18.
