Homeless and Asylum Health Team

The Homeless and Asylum Health Team offers access to primary health care to socially excluded, marginalised and vulnerable groups.
A full health visiting service is provided to women's refuge, housing association projects, hostels and other accommodation. Health assessments of asylum seekers and refugees are completed to support access to health care for all age groups.
The team is made up of health visitors, a health advocate and a nursery nurse. The team works in close partnership with the Housing Department and other statutory and voluntary sector organisations to provide coordinated care to clients.

Homeless people can be referred to the team by any agency, they can self-refer, or they can access this service direct at drop-ins, health sessions and groups, or by contacting the team to set up an appointment or home/hostel visit. More details in the attached Information Sheet.
