Evolve

Evolve offer intensive support to young people and young adults (up to age 25) who are at risk of, or affected by:

• going missing
• child exploitation
• gang
• serious youth violence

Their work centres on the importance of creating safety in relationships - with a primary carer, trusted adult or consistent professional. Evolve can offer non-judgemental and trauma informed intensive support which focuses on what young people’s / young adults’ behaviour is telling them about their lived experience. They offer a variety of direct work which includes:

• 1-2-1 structured sessions
• intensive long-term relationship-based 1-2-1 support
• work with young people and young adults and their parents and carers
• targeted group work
• team around the worker approach, delivering support through the professional with the established trusted relationship

The work is bespoke, consent based, and service user led. This means that the team work hard to find creative ways to engage with all young people who are referred to them.  However, it is not compulsory for young people / young adults that they work with Evolve - they are a non statutory service. Upon successful engagement with young people / young adults who are referred to them, they will work with them for as long as it takes (and beyond).

If  you  know  a  young  person  /  young  adult  who  you  think  would  benefit  from  specialist  ongoing  support  from  the  team  please  complete  a  referral  form  and  exploitation  risk  assessment and send together to evolve@wandsworth.gov.uk

Referrals will be reviewed on a weekly basis and referrers will receive communication from the team about the progress of their referral when all information has been reviewed. Sometimes a different team/service is more appropriate, this will always be communicated to the referrer and they will be assisted with onward signposting. 

Evolve can also support professionals to navigate complex cases via training, consultation, sharing resources and multi-agency meetings and forums. If you would like this kind of support from the team, please get in touch on evolve@wandsworth.gov.uk
