To apply for a Disabled Person's Freedom Pass you need to complete an application form. The application form can be downloaded from Wandsworth Council's website or to ask for one to be put in the post to you, please call 020 8871 8871.

You will need to provide one proof of your name and age and one proof of your current address. Once your eligibility has been confirmed you will be issued with a Freedom Pass.
