Wandsworth Children's Continuing Care Team (WCCCT)
The Wandsworth Children Continuing Care Team (WCCCT) is a flexible, culturally sensitive, home-based care service for children and young people with a life-limiting illness, supporting them and their families.
This service is for Wandsworth residents.
The service provides:
- nursing care in the home, given by trained children's nurses and healthcare assistants; and
- support for the families of life-limited children and young people.
Anyone can refer themselves to the team for an assessment of eligibility. Please contact them to discuss further.
